It's the balloon making you think of Diddy Kong, like a cross between that and Micro Maniacs, though i've never played it.

Most of the levels are from a standard mario-kart viewpoint, but there are also 'adventure' stages like the one shown where you do some platforming in your kart. Great game and one of the best 'karters' around IMO.

BTW, I have to say, when choosing a screenshot, I truly have no idea which would be considered tricky, and which are dead easy. It's surprising how quickly some of these things are answered.

It's known as Sink or Swim on the Amiga, but that looks like the Megadrive version which is called S.S. Lucifer - Man Overboard.
