Blinded in Project Zero,
The Yoshimitsu lookalike Masked Man, and Stephanie in Clock Tower 3
There was a Giant Centipede on one of the Resident Evils that creeped me out, same with the half woman/Half Centipede boss on Kuon
Frankinstein JNR on the Commodore 64 used to creeped me out for some reason I remember a dark room
Shadow of the Colossus last boss for the size of him

Pyramid head from Silent Hill 2. He inspired a sheer pant wetting terror that also made me want a chunk of Toblerone...

I think the Redead in Twilight Princess are even worse.

Those two dogs that jump through the window right in front of the camera not long into Resident Evil scared my brother so badly he wouldn't play the game again. But those frog-men that lollop around, then suddenly leap forward and slice at you make me jump.

Mudmen in Moonstone always made me jump.

I think it was the noise that accompanied them.

The pile of rocks that came alive in Dungeon Master made me jump out of my skin . . . .

There's something horrible about Tartarus' Reaper/Death enemy in Persona 3. You get a warning that Death is nearby and then all you hear is the clanking of chains. What makes it all the more unnerving is that Tartarus is like a maze so you're never sure if he's suddenly going to be around the next corner and it's a frantic scramble for the stairs before he catches up...

Near the end of the game, you can kill him though so it's not as worrying. I had to be cheap and use the 'Armageddon' fusion ability though.

The Cyber Demon in Doom 2, stomp stomp stomp

Not really an enemy as such, but the mannequins in the department store in Condemned shat me right up. Easily the most scared I've ever been while playing games.
