Mine would be:

Fallout 3 (200-odd hours) - utter utter genius.
Phantasy Star Online (probably just as much, though much of it was spent chatting)
Pokemon Yellow - which I played to death at the height of Pokemania, despite being way too old
Ridge Racer Type 4 - loved the atmosphere and played it again and again - Jogcon rules.
Tetris (Gameboy) - addictive and portable - all you needed in 89.

02) Final Fantasy XII (120 hours)
03) Fallout 3 (100 hours)
04) Elder Scrolls IV: Oblivion (80 hours)
05) Shin Megami Tensei: Persona 4 (75 hours)

Fairly recent games. Older games just never seemed to pull me in for these amounts of time. FF6 and Chrono Trigger used to be the top two on my list, but they were both in the 55 hour range.

In no particular order...

1. Final Fantasy VIII
Restarted once out of choice and also restarted again when I accidentally erased my save data half-way through the game. Had about three files in all, each with around 50-80 hours easily clocked up.

2. Pokemon Series.
I had 220 hours+ (yikes!) clocked up on Sapphire alone so when you think that I've played every generation of the handheld games extensively plus many of the spin-offs...I'd rather not know how long it's been exactly.

3. SMT Persona 3/Persona 3 FES
Had over a 100 hours clocked up on the original then another 130+ on the FES version.

4. GTA San Andreas
Always played several times a year for many weeks before I get bored again. Simply too much fun on one disc.

5. Final Fantasy Tactics Advance
I seem to remember having over 200 hours on the save file for this game before I finished it.

There's loads more that I can't think of. Counting the endless replays, I've probably played the PS1 Spyro games to utter death over the years. Then there's the Timesplitters games which I never bore of and periodically play over and over.

May not be 100% accurate but:
1) Pokemon Diamond (those 515 hours flew by)
2) Pokemon Sliver (That game was my childhood)
3) Pokemon Ruby
4) Final Fantasy 7 ( Its one of those games where you get far, stop playing, forget about it, then start again)
5) Not sure but I'll put my money on Final Fantasy 8 or 9.

I don't play those games much anymore but I have played them a lot in the last 8 years.
