Im itching to play through flasback (md) again. So I would love to see that get the go ahead.
Mega lo mania is also a classic.
Another more obscure choice imo is buck rogers "count down to doomsday" on the md. It's a cracking game and well worth anyones time.
I also noticed that someone on here mentioned ninja spirit (on the PCE i presume) which is truly superb.
Or what about moto roader?
Or even better, Final Match Tennis!

Though the game last time wasn't for me, trying to link it in with an "ultimate" type article would be great. Though obviously the page count dedicated to one game/series may get too much and seems to have got mixed response.
