Heart Of Darkness on PS1. I don't think any other game kills a kid in so many unpleasant but entertaining ways.
If it's slower than me, dumber than me and it tastes good ... tough titty.
Green Beret. Stabbing enemies with a knife. That's one game that would have been debated in Parliament if knife crime was as bad as it is today.

The big ones for me as a kid was Another World. How much blood can a stalgmite get out of you after the 30th death?!
Another vote for Gloom on the Amiga (CD32) version.
Solider of Fortune (pc) - The first game to give the ability to target your enemy in any particular area of their body, resulting in a dismemberment of your choosing. Being able to shoot off certain limbs off an enemy in real time, was a huge graphical leap in mature gaming, yet surprisingly it's never been replicated.
I don't know if it's "Retro", but I say the Clock Tower serie..scaring especially when you try to hide and Scissorsman finds you. I was no longer a child, but trust me I literally jumped over the chair.
Not sure how retro it is, but Eternal Darkness on the Gamecube. Many of the characters meet gory and horrible ends, Paul gets stepped on and squashed by the black guardian, Roberto gets buried alive as part of the pillar of flesh, not to mention the sanity affects. At one point my characters arms just fell off, I tried to equip a weapon and got a sarcastic message saying it was difficult to do that with no arms, then my character exploded, and I was back where I started.
Nothing beats the gore of the "Oh no!" with all the lemmings exploding at once. Top.

sorry about this... utterly un-retro but Until Dawn on the PS4 has some super gory death scenes.
