I've got no choice but to be extremely boring and go for Streets of Rage 2.

Honourable mentions go to Sonic, Side Pocket, Desert Strike and Road Rash 2...amongst countless others. What a hard question!
Retro is a state of mind, and cares not for your puny concepts like dates and calendars.

Choosing between my favourite two could almost come down to a coin toss, but I'm going to go with...

Buck Rogers: Countdown to Doomsday

It was the first RPG of any type since Wasteland to keep me completely enthralled from start to finish. It meant keeping my youngest brother off his Megadrive while I played it, but that was his own fault for not being old or big enough to shift me. Brilliant game.

Only just beats out the epic Landstalker.

Never played many - was out of the gaming world for a good number of years - but thoroughly enjoyed Devil's Crush (Devil Crash in Japan) on my brother's system. Still remain unbeaten amongst his large group of friends. :)

Desert Strike - I was crap at it (and probably still am) but I just loved rescuing those little guys!

Sonic the Hedgehog 2; this is to me the best Sonic game ever and, ironically, a game that wasn't even made by Sonic Team.

Altered Beast & Wrestle War. The first games I played for Mega Drive. The large sprites and music of both games really was an arcade in the home. Super Real Basketball also for its large sprite cutscenes.

so many great games on the MD, a lot already mentioned, NHLPA93, Golden Axe, Streets of Rage etc.
But the best game, nay, the best RPG ever, has barely got a mention, is it because all the games that came after carrying the name were massive let downs? (in my opinion anyways, still good games tho)
Phantasy Star IV
the adventures of Chaz, Alys, Wren, Rika, Gryz etc is still the benchmark for me

Umm well ill leave the arcade and format conversions and stick to the MD originals for this, the obvious ones I play the most are EA Hockey 93, Desert Strike, Road Rash 1,2, Sonic 1,2,3, Thunderforce 3,4, Revenge of Shinobi, Shinobi 3, Wonderboy in Monster World IV, Streets Of Rage 1,2, Gunstar Heroes...
Since I have to pick one, it goes on the most fun and time spent playing it, then it would have to be Road Rash 2.

I was going to mention that, another in a long line of fantastic titles, but if I did then I would have to mention 2 and 3, then theres the excellent buck Rogers countdown to doomsday, sword of vermillion and d+d warriors of the eternal sun. That's without mentioning the likes of shining force...
